When in a diner....Coffee? (simple question for people from USA)?

In most restaurants coffee is refillable. When they bring out the pot for your mug and they keep refillinging it than it means that it is normally free refills. But if you go up to order from a counter than it's most likely charging you per cup. You should ask because each diner is different.
